GAP Care
For people who either don’t have a Primary Care Physician or are experiencing an unacceptable delay in care, Noble Physicians offers prompt, convenient, and comprehensive follow-up care. “GAP Care” literally closes the gap after an acute hospitalization, urgent care visit, illness, or injury, warmly passing the baton of care to home health providers, specialists, and primary care providers. “GAP Care” providers uniquely aid in obtaining onsite urgent care and facilitate transfers to the Emergency Department when appropriate, helping streamline these challenging moments, reducing stress and improving outcomes.

Mental Health
Noble Physicians offers evaluation and management of the full spectrum of Mental Health needs, from the most common to the most challenging conditions. Our world class providers employ a variety of approaches to care, including medication management and a variety of “talk therapy” approaches. Our clinicians recognize that everyone is unique and what works for one may not work for another. Apart from prescription management and counseling, our providers may prescribe other therapeutic modalities such as EMDR, DBT, and Transcranial Magnetic Stimulation.
